General Policies and Procedures

• I work in a shared facility with limited outside access.  Please text me upon your arrival to the clinic as the door will be locked if I am in session.  Look for the sign on the door.  Parking is to the left of the building.  

• I will ask you to complete an intake form yearly with medical updates.  Massage is a taxable service in the state of Kentucky.  If you ask your medical provider for a prescription for massage therapy, it will eliminate the tax and reduce the price.  The posted prices on the website include sales tax. Prescriptions are good for a calendar year unless otherwise specified.

• If you feel unwell or have another issue that would prevent you from coming to your appointment, please cancel with a minimum 24 hour notice.  This will provide the opportunity for another client to schedule.  You may cancel through the website, but please also text me upon cancellation as a courtesy.

• I accept debit/credit cards, PayPal, Venmo, and cash/personal check.  Payment is due at the time of session.  I also sell gift certificates good for one year from date of purchase.

• Professional massage therapy is considered complementary health care.  As such, no illicit conversation, gestures or activities will be tolerated.
